In Rapid Random Review (RRR) we discussed:
- Thrombocytopenia in newborn
- Congenital Adrenal Hyperplasia (CAH)
- Respiratory Distress Syndrome
The Video station was on a 4 day old baby admitted following referral by midwife and clinically looking jaundiced. Dr R took the hotseat, Dr P was with examiner hat. Dr R noted the clinical findings. She made a diagnosis but could not get a differential of common caused. The history and examination also needed help by observer Dr S.
Dr Shishir gave very valable tips.
The Learning points are:
- Read the initial information carefully and watch FULL video.
- Focus on different aspect of video clip.
- Develop a differential diagnosis of atleast 2-3.
- Common things are common – hence consider first.
- This guides your clinical reasoning – history & examination.
- Sepsis should always be considered and not missed as can swiftly be fatal.
- Weight – current and at birth – is an important marker of hydration in neonate.
- Always examine scalp for haematoma and Anterior Fontanelle.
- Investigations – mention with expected results
- Management is not only about drugs – supportive care equally important.
