MRCPCH CLINICAL: Abdomen. 1-2-1

n RRR we discussed:

  • Hypertension in a 8 year old
  • Deafness in a 6 year old
  • Short stature in 7 year old

The Clinical station we discussed today was Abdomen. It is a common scenario and one you are likely to come across. First was a 15 year old with evidence of a scar and fullness in Right Iliac fossa and second was a 6 yr old child with features suggestive of a Haemolytic Anaemia – likely Thalassaemia. You were in the hotseat for both and managed with a little support. 

There are some Learning points from our session:

  • Do NOT panic – Listen Very carefully to Instructions
  • Think and Talk about the child / patient in front of you
  • Do not give generic answers – as if talking from a book
  • Common things Common – should be mentioned first
  • Give Diagnosis first – if you are confident instead of narrating findings
  • Support by +ve findings and important -ve’s 
  • Remember Examiner is watching your technique very carefully
  • Presence of mind is necessary – ‘think on your feet’
  • Corelate your observations better to reach a Conclusion
  • e.g. Steroid tablets with Renal mass – ? transplant and immunosuppression
  • Know broad management steps for common conditions
  • Management – is a TEAM effort so involve others prn

We then discussed How to attempt the Video station and will practice at our next session.
